The Nurse Practitioner (NP) in the Community & Population Health Program will be registered with the College of Nurses of Ontario in the Extended Class – Adult and work within the scope of practice of the RN-EC. As a member of the Centralized Care and Transition Team (CCaTT), the NP will work within an interdisciplinary team and with the Emergency Department team utilizing a patient and family centered model of care. The CCaTT NP will be responsible to provide direct patient care including: assessment and treatment of patients with multi-morbidity often impacted by social determinants of health and frailty; develop integrated care plans supporting seamless transition from hospital to community. Additionally, the NP will tutor and mentor nursing and other health professional students as appropriate; participate in continuing education activities to integrate the role of the NP in the program and academic settings; initiate, participate, and/or lead programs, presentations, publications, evaluations and other system changes to continue the enhancement of interdisciplinary care teams. The CCaTT NP will be instrumental in working with Program Leadership and stakeholders to develop and execute strategic plans, and to advance best practices for operational excellence and sustainability.
Salary: 49.69-63.71
