The Clinical Pharmacist is a member of the multidisciplinary healthcare team providing pharmaceutical care to patients at Hamilton Health Sciences – Juravinski Hospital.
The pharmacist will provide care to individuals with malignant hematological diseases including blood and marrow transplants, acute medicine, surgical oncology, orthopedics, seniors, Emergency and Community Medicine Programs.
The Pharmacist will participate in interdisciplinary patient care rounds, provide patient-centered medication therapy management and make appropriate evidence-based recommendations as part of the interdisciplinary team and document interventions; Actively participates in the development and implementation of performance/quality improvement and patient safety activities for clinical service area; Participates in mentorship and professional development of Clinical staff including pharmacy residents and pharmacy students. Precept residents and students in rotations of area of specialization. Engages in professional self-development; Collaborate with care teams to develop intervention strategies to maximize medication utilization, including but not limited to evidence based cost effective medication guidelines, clinical pathways, core measures, best practice guidelines, and policy development related to medication management in critical care. The Pharmacist reviews and assesses medication orders and works with the patient and healthcare team directly to optimize medication use, avoid mediation-related problems and achieve desired patient outcomes. The pharmacist will provide drug information, will promote evidence-based/cost effective drug therapy, will promote clinically relevant research and be involved in educational activities for students, families and staff. Ongoing collaboration and communication within the healthcare team and with the pharmacy team is essential.
Salary: 44.49-57.04
