Pharmacist

The Pharmacist is a member of the multidisciplinary healthcare team that provides pharmaceutical care to patients at Hamilton Health Sciences. The Pharmacist reviews and assesses medication therapies and works with the patient and the healthcare team directly to optimize medication use and minimize medication-related problems to achieve desired patient outcomes.  This work may be performed on individual clinical units (primarily in the Emergency Department, with a focus on adult trauma, burns, cardiology and cardiovascular programs, along with all other typical adult and pediatric emergency room care), or may be across multiple units to support the medication system needs of the site.
