This is a Contract position in Hamilton, ON posted August 2, 2022.

The Diabetes Care and Research Program (DCRP) is a subspecialty clinic located within the Boris Clinic, at the MUMC Site. The DCRP provides multidisciplinary evidence-based diabetes education and management for adults with diabetes in an ambulatory setting. In addition to providing tertiary outpatient diabetes care for complex patients, it supports patients collaboratively with primary care physicians. The DCRP’s primary goals are to enhance the quality of life of individuals and their families by improving glucose control, preventing the development of diabetes-related complications, and providing opportunities for their involvement in research to find new ways of preventing and treating diabetes and its complications.

As a member of the interdisciplinary health care team, the Social Worker provides direct patient and family care through the provision of psycho-social screening and assessments; individual and family counseling; crisis intervention; bereavement counseling; liaison and referral to community agencies; advocacy; case management and discharge planning. The Social Worker collaborates with other team members, participates in program development, and participates in professional education and research.

1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
2. Masters degree in Social Work from an accredited School of Social Work
3. Recent (3-5) years experience providing services in an acute health care environment
4. Demonstrated knowledge of Regional community support services
5. Working knowledge of the Healthcare Consent Act and Substitute Decisions Act
6. Demonstrated skills in the performance of a therapeutic role including strong assessment, counseling, case management, crisis intervention and advocacy skills
7. Proven ability to work in a fast-paced, complex environment and to organize, prioritize and problem-solve effectively
8. Strong interpersonal, communication, teamwork, consultation and teaching skills
9. Demonstrated commitment to ongoing learning of self and others
10. Qualified as a field instructor for social work students and demonstrated experience supervising students
