Unit Summary The Pediatric Ambulatory Program at McMaster Children’s Hospital provides outpatient care to over 100,000 children from Hamilton and the surrounding area. It is comprised of numerous sub-specialty services that facilitate the unique health care needs of children using a family centered model of care.

Position Summary

In collaboration with the sub-specialty team, the System Navigator will facilitate care coordination to patients and families in the Ambulatory setting. They will assist the clinical team in the care delivery process by assessing patient needs and goals around social determinants of health, and by helping to navigate families through the system. They will work closely with internal and external care providers to ensure patients and families experience timely access to care, and to the individualized resources that they need. The System Navigator will also assist the clinical team in the development of quality and process improvement initiatives, as well as participate in the establishment of service specific goals and objectives.

Schedule Work Hours

Monday – Friday; Days

This position is temporary until 2022-12-31


1. University Degree in Health or Social Sciences field; or College Diploma with three (3) years’ work experience in the Health and/or Social Sciences field
2. Professional designation is an asset
3. Experience working with, and extensive knowledge of the local health care system and community resources related to the pediatric patient population.
4. Excellent interpersonal, oral and written communication skills
5. Proven ability to work as a team member
6. Demonstrated ability to exercise sound judgement and decision making skills
7. Experience with quality improvement initiatives related to care delivery processes
8. Demonstrated initiative and experience working independently with minimal supervision
9. Proficiency with Microsoft Office programs, internet research, EMR and ability to learn and master other computer programs as needed (i.e Meditech, Clinical Connect)
