MSW-Social Worker

Grand River Hospital

This is a Contract position in Hamilton, ON posted October 4, 2022.

Location: Freeport Campus 

Work Type: Part Time 

Shift: Monday – Friday (3 days per week) 7.5 Hour Shifts

Grand River Hospital’s Complex Medical/Chronic Assisted Ventilator and Palliative Care Inpatient Programs are available at the Freeport Campus. Reporting to the Clinical Manager, the Social Worker is a highly motivated, resilient and an adaptable professional, with advanced clinical and critical thinking skills. Success in the role requires you to demonstrate an ability to foster a culture of compassion through caring relationships, leading practices, specialized services and partnerships. You are a strong leader with extensive clinical experience in family systems, coupled with the ability to be highly motivated and work effectively within a multidisciplinary team environment. 



  • Master of Social Work degree with registration in good standing with OCSWSSW;
  • Highly developed psychosocial assessment and counselling skills that are attachment, grief and trauma informed care.
  • Required experience in delivering brief models of therapy and family therapy work.
  • Required minimum of 5 years related experience post graduate degree in mental health, hospice/hospital or EOL care and grief support.
  • Demonstrated advanced skill in crisis management and containment.
  • Demonstrated ability to communicate effectively both verbally and in writing with clients, families, other members of the health care team, and community agencies.
  • Proven ability to work effectively with other health care providers and community service providers.
  • Excellent organizational, prioritization and problem-solving abilities;
  • Proven to have strong interpersonal skills and written and verbal communication skills; positive attitude.
  • Good attendance record.
