Transitional Case Management Supervisor
– Contract Mission Statement: Homes First Society develops and provides affordable, stable housing and support services to break the cycle of homelessness for people with the fewest housing options.

Hours of Work: 5 days per week, flexible schedule (weekdays, evenings and weekends) Contract Period: ASAP until December 31 2021, program extension is expected.

APPLICANTS MUST QUOTE “TCMS” This role is part of a multi-disciplinary team skilled in de-escalation and crisis intervention, providing street outreach in two Downtown East Areas to help build safe and inclusive communities.

The team as a whole will provide consistent, rapid response, short-term follow-up supports, and outreach to people experiencing marginalization.

This role will provide short term follow-up support and be available for rapid response calls.

Additionally, this role provides transitional case management to address individual needs for follow-up support including making referrals for mental health, substance use, system navigation and transitions, housing, and health care.

The focus is on immediate intervention with clients; which can include de-escalate, perform wellness checks, crisis counselling, risk assessments and nonviolent conflict resolution.

The project will incorporate a trauma informed understanding of marginalized communities including a strong analysis of anti-Indigenous and anti-Black racism in all aspects of the project from hiring, project development to implementation and data collection.

[this is part of the job description] Job Description: Accept client referrals from the multi-disciplinary team and provide client-centered follow-up support to help clients stabilize and connect to community-based supports and services including making referrals for mental health, substance use, system navigation and transitions, housing, and health care.

Complete assessments of support needs to provide referrals to a wide array of community-based supports, services and programs based on clients goals, needs and choices Incorporate a trauma informed understanding of marginalized communities including analysis of anti-Indigenous and anti-Black racism from hiring , project development to implementation and data collection Develop a client crisis plans when necessary.

Provide immediate wrap-around case management supports and communicate with landlords to stabilize housing when necessary or provide rehousing supports and assistance where tenancy could not be sustained.

Participate in daily team meetings within the multidisciplinary team Maintain strong practice of privacy and confidentiality.

Travel within the GTA as necessary to support clients, team and attend various meetings.

Directly report to Homes First Supervisor and work in conjunction with multidisciplinary team’s Coordinator Interact with project the Steering Committee and the Management Team as appropriate Participate in community Outreach to build rapport, education, and referrals to teams services Document and maintain client records professionally and confidentially in accordance with Homes First standards Complete and continue ongoing training workshops to strengthen competency in the following areas: de-escalation, including Non-violent Crisis Intervention and Prevention (CPI); suicide intervention (ASIST); conflict resolution; cultural competency, anti-racism training focused on working with Black and Indigenous individuals and anti-oppression competencies; first aid and CPR; and overdose response and Naloxone training .

Collaborate with existing community agencies and outreach teams to determine and fill gaps in existing service as well as ensure minimal duplication of services.

Participate in community Outreach for rapport building, education, and referrals.

Qualifications: Post-secondary degree in Mental Health and Substance-use or a related field; or an equivalent combination of education, training and experience Three years of counselling experience Proven interviewing and counselling skills Demonstrated understanding of the psychology of substance-use, issues that underlie substance-use issues, trauma and mental health issues Demonstrated case management experience Exceptional communication skills – written, verbal, and interpersonal Able to work independently and in interdependent teams, with minimum supervision in a busy and flexible environment Mobile and comfortable delivering care in outdoor environments and possibly clients’ homes Proficient in Microsoft Word and Outlook Experience working in a supportive housing setting an asset Certificate in First Aid, CPR an asset Cultural-sensitivity training an asset Excellent organization and time management skills Ability to work evenings and weekends if necessary Homes First Society is an equal opportunity employer Working in an environment where there may be smoke is required.
